Your First Visit - Making an Appointment
Often, your first visit with a surgeon is the most important.
To provide quality care and proper diagnosis, you should bring
the following with you to your first visit with the surgeon:
- Any and all insurance cards and documents including your major
health plan and supplements.
- Sometimes, your insurance plan may require a referral from
your primary care doctor. If this is the case, please bring
this document with you. If a referral is required and not arranged
through your primary care office with proper documentation we
will have to reschedule your appointment.
- Please bring any and all prior diagnostic scans including
EKG, holter monitor studies, etc. with you to your first appointment.
These diagnostic tests and results are essential for the surgeon
to properly diagnose and treat you. Without these test results,
we will have to reschedule your appointment.
- Please bring a complete list of all medications (both prescribed
and over-the-counter) you are currently taking. Along with the
list of medications, please include the dosage and frequency.
- Be sure to let the surgeon and staff know of any medication/health
allergies you may have.
- List of past procedures and surgeries
- You may find it helpful to bring a list of prepared questions
and concerns you want to be sure and address with your surgeon.
Preparing your thoughts and questions before the visit can help
you make the most of the time you have with your surgeon.
